Public (GKV) cost depends on your salary, while Private (PKV) depends on your age and health status. Public covers families for free; Private charges per person.
Every resident. Employees earning below €77,400 (2026 limit) must stay in Public. Freelancers and high-earners can choose to opt-out to Private.
If you earn above the JAEG limit, Private may be cheaper and offer better care. If you are below, Public is mandatory and automatically deducted from your payslip.
